Background

In Hong Kong there are about 22,000 stroke rehabilitees discharged from hospitals each year. Based on this figure, 60 families will be affected each day. With the support of the Lee Hysan Foundation and the Sau Po Centre on Ageing, The University of Hong Kong, the “WeRISE - Stroke Family Empowerment Project” scheme has been launched in collaboration with the medical and social service units in Southern, Shatin and Tuen Mun districts, dedicated to supporting families of stroke patients and helping them to re-integrate into the community.

Vision

We believe that each family caring for stroke rehabilitees has the potential resources and strengths to help the person recover. As long as short-term, post-discharge assistance is provided, families of stroke patients are able to strengthen their functions to develop sound communication, regaining faith in accepting the challenges in a post-stroke life while adjusting to their new lifestyle.

Strategy

Family-Oriented, Medical-Social Collaboration (Strategic Partners)

Strokes affect the whole family. Therefore, this scheme provides family-oriented services to help families of stroke patients restore family functions and ‘rise’ again. Through close cooperation with hospitals and social welfare organisations, families of stroke patients can access seamless care and support.

Led by Social Workers, Collaborated with Volunteers (Dual Case Management and Support Model)

Under this scheme, senior social workers (case managers) provide families of stroke patients with care management services, and district senior volunteers (escorts) give peer support. With this dual case management and support model, we aim to provide families of stroke patients with comprehensive support.
