This course is produced by the ‘WeRISE’ team of the Sau Po Centre on Ageing, The University of Hong Kong. It aims to enrich the understanding of and concerns about stroke care of carers in families with stroke patients, service providers, and other organisations. The course consists of four periods: ‘Preparation for Discharge’, ‘Care Advice’, ‘Family Collaboration’, and ‘Community Integration’.
