WeRISE
Stroke Family Empowerment Project

"WeRISE - Stroke Family Empowerment Project" has been dedicated to supporting stroke families since 2016. From July 2016 to March 2020, WeRISE has collaborated with the Aberdeen Kaifong Welfare Association Social Service, Evangelical Lutheran Church Social Service - Hong Kong, and the Neighbourhood Advice-Action Council to develop a standardized intervention model for stroke family support led by care managers and volunteers. The trained support team has provided more than 2,100 hours of support to 200 stroke families and achieved remarkable results. We would like to express our gratitude for the support and encouragement from stakeholders. At present, WeRISE continues to provide support for stroke families in innovative ways. Educational resources are freely available to stroke families and the public. You are welcome to browse the Resources Booklet and Self-learning videos through the WeRISE website. Also, you are encouraged to sign up for a Certified eLearning course which is free of charge. At the same time, you can download the WeRISE App to actively manage your health through technology and prevent secondary stroke. Professionals are welcome to share our educational resources and WeRISE App with stroke families. Social workers and other healthcare professionals can contact us to arrange standardized care manager training and obtain the Care Manager Manual.
